A new program.
A familiar goal.
Looking for information about a Home Care Package? The name has changed, but understanding your own care and next steps still matters.

Which situation sounds like yours?
I am new to aged care
Start with current Support at Home information rather than an application for one of the four former packages.
Explore Support at HomeI have older HCP paperwork
Use your current letter and care agreement to check your transitioned classification and arrangements.
Understand the terminologyMy needs have changed
Tell your care team what is different. Ask whether the care plan needs review or a reassessment is appropriate.
Talk to HomeCaringTHE FOUR FORMER LEVELS
Recognise the wording
on an older letter.
Historical descriptions only. These are not the eight current Support at Home classifications.
KEEP THE CONVERSATION PRACTICAL
Your transition checklist.
Find your current documents
Keep the latest outcome, fee advice and service agreement together. Older letters can help explain the history, but may not describe today’s arrangement.
Check the plan
Ask how your services and budget are recorded now. Raise any unfamiliar wording or changes before assuming it is the same as your old package.
Ask about continuity
If changing provider, discuss the handover and start date with both teams. Make a plan for essential support during the change.
LET’S FIND YOUR NEXT STEP
You don’t have to decode it alone.
Tell us whether you are new to care, reading an older letter or thinking about changing providers. We can start from there.
