A CONVERSATION ABOUT YOUR NEEDS

A strong plan makes responsibilities visible.

01

Personal priorities

Begin with the person’s preferences, daily routine and how they want to be involved in decisions.

02

Day-to-day care

Talk through essential support and the practical arrangements for delivering it.

03

Clinical coordination

Discuss how relevant health professionals and the care team will share information with consent.

04

Review and changes

Ask who to contact if needs change and how the plan will be reviewed.

MAKE IT PERSONAL

Ask about the gaps as well as the visits.

Know the plan.
Know who to call.

Discuss what happens outside scheduled support, how missed visits are managed and who can respond to a change in need. Do not assume a former Level 4 budget covers continuous or round-the-clock care.

These are discussion prompts, not a service entitlement or a recommended clinical care plan.

THEN AND NOW

Keep the old label
in perspective.

USEFUL TO KNOW

Questions about Level 4.

Does Level 4 mean 24-hour care is funded?

No. The former highest HCP level was not a guarantee of continuous care. Discuss the actual schedule, costs and any unmet needs with your provider.

Can a care plan replace urgent medical help?

No. Care planning is not an emergency service. In an emergency, call 000.
