FORMER HOME CARE PACKAGE · LEVEL 2
Support for
everyday rhythms.
The former Level 2 package was associated with low-level care needs. Use this guide to connect that older label with the support you need now.
Explore current supportA CONVERSATION ABOUT YOUR NEEDS
Look at the whole week, not just a single task.
A comfortable morning
Think about getting ready, meals and the routines that help the day begin well.
A manageable home
Talk about housework, shopping and the tasks that are becoming difficult.
A connected week
Consider appointments, community activities and the people you want to keep seeing.
MAKE IT PERSONAL
A good plan should fit around your life.
Familiar routines.
Thoughtful support.
Bring a simple outline of your usual week. Note what family or friends already help with, where you would prefer more independence and any times that do not work for visits. These preferences help make the conversation specific.
These are discussion prompts, not a service entitlement or a recommended clinical care plan.
THEN AND NOW
Keep the old label
in perspective.

Check your own arrangements
Read your latest outcome and agreement. The same number in Support at Home does not automatically mean the same funding or services.
Understand current classificationsUSEFUL TO KNOW
Questions about Level 2.
Is a transitioned Level 2 the same as Classification 2?
No. Transitioned HCP classifications and the eight ongoing Support at Home classifications are different. Check your current outcome letter for the wording that applies to you.
Can support include different types of help?
Discuss your assessed needs and approved services with your provider. A historical level alone does not establish which services are available today.
