A CONVERSATION ABOUT YOUR NEEDS

Start with the small things that make a day easier.

01

At home

Which household tasks are taking more energy than they used to?

02

Getting ready

Is there anything in your personal routine you would like help with?

03

Staying connected

Are shopping, appointments or seeing friends becoming harder to organise?

MAKE IT PERSONAL

Choose your priorities, not a ready-made bundle.

Your priorities.
Your pace.

Write down the tasks you would most like help with, then mark the one or two that make the biggest difference to your day. Use this as a conversation starter, rather than a shopping list of services.

These are discussion prompts, not a service entitlement or a recommended clinical care plan.

THEN AND NOW

Keep the old label
in perspective.

CURRENT NEXT STEP

Check your own arrangements

Read your latest outcome and agreement. The same number in Support at Home does not automatically mean the same funding or services.

USEFUL TO KNOW

Questions about Level 1.

Does a former Level 1 mean one visit each week?

No fixed visit schedule should be assumed from a historical level. Your current plan, service prices and funding determine the support you can arrange.

Can I apply for a Level 1 package now?

New applicants should use the current My Aged Care assessment pathway. Home Care Packages have been replaced by Support at Home.
