Support at Home

IN-HOME AGED CARE

Support at Home.
Life, your way.

Understand your options for funded care, then build support around the routines, relationships and place you call home.

Talk about your care
Support at Home has eight ongoing classifications and three short-term pathways. Your assessment guides the next step.Support at HomeCare shaped around you8Ongoing classifications3Short-term pathwaysYour assessment guides the next step

A CLEARER START

Care starts with you.

01

Talk about your needs

A conversation about what has become difficult is a useful place to begin. Include the things you want to keep doing, not only the tasks you need help with.

02

Understand your assessment

My Aged Care is the starting point for government-funded aged care. Assessment helps establish your support needs and the services you may be approved to receive.

03

Agree your care

With your outcome and available funding, compare providers and talk through a care plan, service prices and your preferences before you begin.

MORE THAN ONE PATH

Ongoing support.
Focused help, too.

8

Ongoing classifications

Different assessed needs are supported through eight funding classifications. Your outcome guides the plan.

Understand classifications
Assistive technology & home modifications
Equipment and changes to the home have a separate pathway, subject to assessment and approval.
Restorative care
A short-term pathway focused on regaining function and independence.
End-of-life care
A dedicated pathway for eligible people who wish to receive care at home near the end of life.

PRACTICAL CONVERSATIONS

Make room for what matters.

Your routines
Which parts of your day are comfortable, and which need a little more support?
Your people
Who would you like involved in planning, and how should the team communicate with them?
Your budget
Ask for current service prices and a clear explanation of any contributions before agreeing to services.

QUESTIONS, ANSWERED

A few useful distinctions.

Is this the same as a Home Care Package?

Support at Home replaced Home Care Packages on 1 November 2025. Existing HCP recipients have transitioned arrangements; a former HCP level is not automatically the same number in the new eight-classification system.

What about the Commonwealth Home Support Program?

CHSP remains a separate program. My Aged Care can explain which program applies to your assessed needs.

Can I talk to HomeCaring before I have an assessment?

You can contact the team to discuss care and the next steps. A provider conversation does not replace the government assessment process.

LET’S FIND YOUR NEXT STEP

Let’s make care feel clearer.

Bring your questions, your assessment outcome if you have one, and a sense of what a good day looks like for you.
